If you are registering a teen (6th-12th grade) who will be dropped off without a parent or guardian present, please provide the name and contact info of the person responsible for drop off and pick up of your child.

Adventure of Faith church photographs all of our special events and programs for the purpose of celebrating with our congregation, sharing in our newsletter, social media account, advertising, etc. By checking the box signed “Media Release”, you understand that photos containing images of you and/or your family may be used by Adventure of Faith Church for our website, social media, newsletter, and other media. *If you have a situation that prevents you or your family from being in photos used by Adventure of Faith Church, please email mainoffice@faithadventure.com

Media Release

In case of emergency, accident, allergic reaction, or serious illness, I hereby authorize, by checking the “Emergency Services Allowed” box Adventure of Faith and its agents to perform any acts which may be necessary or proper to provide emergency health care for me and my child(ren). This is including but not limited to consent to and authorization of medical procedures by emergency medical technicians, physicians, dentists, hospital or other emergency medical personnel, as they exercise what at their sole discretion, they deem necessary. Furthermore, by checking the “Emergency Services Allowed” box the individual/parent/guardian understands that they are responsible for all costs and expense of such medical treatment.

We’re excited to have you and your family join us at BaseCamp! Please take a few minutes to complete this form so we can get to know you and your family and prepare for a great session together.

Cost & Payment
The suggested cost for BaseCamp is $5 per person up to $20 per family of four or more per night. Payment information and instructions will be included in your confirmation email after you register.
We want BaseCamp to be accessible to every family who wants to participate. Please don’t let cost keep you from coming. If the suggested cost would be a barrier for your family, scholarships and financial assistance are available. Simply email candice@faithadventure.com, and we’ll help you with the next steps.

Help Another Family Attend
BaseCamp is made possible in part through the generosity of people in our church family. If you are able and would like to help another individual or family attend, you can give toward BaseCamp Scholarships at any time throughout the year. Your generosity helps ensure that finances don’t prevent someone from being part of this experience.

Whether you are paying the suggested cost, need some assistance, or can give a little extra to help someone else, we want you at BaseCamp!
