Youth Summer Camp 2024 Registration Form (1 per student)

August 11th through August 14th | Our yearly trip to summer camp is a great time of team building and connecting for our teens and leaders in Adventure Youth. When we strip away technology, and take kids out of their regular context, it makes for an awesome opportunity to connect with God and one another.

LOGISTICS:
Camp will be from Sunday, August 11th through Wednesday, August 14th at Black Diamond Camp in Auburn, WA. We will meet at the church at 2pm (subject to change, please watch for info closer to camp dates) on August 11th and leave after a time of prayer and info share. While we will have some youth leaders attending who have room in their vehicles, we will need the help of parents/guardians to transport teens to and from camp.

On Wednesday, August 14th, students will have breakfast and have a checkout time of 10am. Students can be picked up that day at camp or will return back to the church at approximately 11am if riding with a leader or other youth parent. All times will be confirmed and communicated closer to camp dates.

Black Diamond Camp doesn't allow students to bring electronic devices, so we kindly request that students leave their cell phones, smart watches, and other electronic devices at home. We want this to be an opportunity to unplug for the week. If there is an emergency while at camp, feel free to contact Callie Williamson on her cell: 360-865-3193. Callie will make her best effort to give periodic updates of how things are going at camp.

Black Diamond Camp has a separate registration form that you will be required to fill out, this will include further information regarding medical and dietary needs. You will receive the link to complete the Black Diamond registration in the confirmation email sent after completing this form.

MONEY:
The cost of camp is $318 per person. At this time, we are hosting multiple fundraisers to lower the overall cost of camp for every teen. If you believe that you will need financial assistance, please contact Callie@faithadventure.com as soon as possible.

Because the cost per person will not be determined until after our fundraisers are complete, we will communicate all payment info to you separately. Payment will not be due until the week before camp.

In order to attend, the following must be completed:
1. Completion of this registration form
2. Completion of Black Diamond Camp registration form (will be emailed to you upon completion of this form)
3. Payment made in full

I understand that the staff and volunteers of ADVENTURE YOUTH and ADVENTURE OF FAITH are not medical professionals. They are not responsible for the timely distribuion of my student's medication. While they will store medicine and provide medication at designated times, my student is ultimately responsible for taking his/her own medication in a timely manner when it is administered.

I release ADVENTURE OF FAITH CHURCH, including its trustees, employees and agents from my students physical injury, including death, or illness while attending or participating in the event(s) or activities. I will assume the risk associated therewith, whether known or unknown to me at this time. This release is also intended to include all claims of my family, estate, heirs, personal represntatives or assigns. 



Authorization of Treatment: I hereby give permission to the medical personnel selected by ADVENTURE OF FAITH CHURCH or it's employees to secure and administer treatment and to maintain and/or release any medical records necessary for insurance purposes as outlined under the HIPAA regulation and to provide or arrange necessary related transportation for the above named person.



I verify that my child is in good health and is capable of participating in strenuous activities, and when necessary, will take his/her activities to those within the bounds of his/her physical health. I recognize that any medical treatment that is provided to my child while attending this event will be paid for by me or my medical insurance company.
